General Information
An applicant may be issued a license by endorsement provided the applicant meets the requirements of A or B below:
License by Endorsement A
The Board may issue a license by endorsement to a practitioner who is duly licensed, certified, or registered as a massage and bodywork therapist under the laws of another jurisdiction, if all documents listed below are submitted and requirements are met:

A completed License Application signed and notarized

License fee and State Bureau of Investigation criminal history records check fee

Executed Fingerprint Card with signed Consent Form for criminal history records check

Photograph
-
Documentation of high school education or equivalent

Documentation of age

You must provide documentation that you currently hold a valid license, certificate, or registration as a massage and bodywork therapist in another state or jurisdiction and that your license is in good standing, with no disciplinary proceeding or unresolved complaint pending. A copy of your current license, certificate, or registration as a massage and bodywork therapist in another state or jurisdiction and a Verification of Licensure is required.

You must provide documentation that the state or jurisdiction's requirements for licensure, certification, or registration as a massage and bodywork therapist are substantially equivalent to or exceed the requirements for licensure under this Article.

You must provide documentation of graduation from a massage and bodywork therapy school that offers a curriculum that meets or is equivalent to the standards set forth in the Board’s criteria for school approval and is licensed or approved by the regulatory authority for schools of massage and bodywork therapy in the state, province, territory, or country in which it operates or is exempt by law. The required documents are an official sealed transcript from your massage therapy school

You must provide four (4) Statements of Moral Character and Professional Competence.

Documentation of proficiency in the English language. If there is a question whether you have a proficiency in English, you shall take the “English as a Second Language” course and submit the test scores with your Application. The Board may request, on a case-by-case basis, that you come to the Board office for an interview with the License Standards Committee.

Proof of Citizenship. If you are NOT a U.S. citizen, you must provide a copy of your green card or visa to document your eligibility for employment in the United States. Please note any documents submitted to the Board in any language other than English must be accompanied by a certified translation.
License by Endorsement B
The Board may issue a license by endorsement to a practitioner from another state that does not license, certify, or register massage and bodywork therapist if all of the following qualifications are met :

Executed Fingerprint Card with signed Consent Form for criminal history records check

Documentation of high school education or equivalent

Documentation of age

Documentation of Competency Assessment Examination

You must submit a copy of your official score report from one of the following exams: (a) NCETMB, (b) NCETM, (c) NCETMB – NESL option, (d) NCETM – NESL option (e) ABTE or (f) MBLEx

If you have taken the exam (NCETMB, NCETM, ABTE or MBLEx) recently and have not received your official score report from the NCBTMB, NCCAOM, or FSMTB, you may request an “Official Score Report” from the appropriate testing service to be sent directly to the Board. Please note the testing service charges a fee for this procedure.

You must provide documentation of graduation from a massage and bodywork therapy school that offers a curriculum that meets or is equivalent to the standards set forth in the Board’s criteria for school approval and is licensed or approved by the regulatory authority for schools of massage and bodywork therapy in the state, province, territory, or country in which it operates or is exempt by law. The required documents are:
- Official sealed transcript from your massage therapy school
- Copy of the massage school's license to operate
- Copy of your massage school's catalog

Copy of your massage school's program curriculum, indicating course descriptions and the number of classroom hours of supervised instruction for each course of the massage therapy program you attended.

Affidavit of Professional Training (Form 05) for graduates of schools no longer in existence.

If you cannot meet the requirements of an official massage school transcript, school's license to operate, school catalog, school program curriculum and the documentation of a competency assessment examination listed in the requirements for licensure by endorsement B, the applicant may provide other credentials, to be reviewed by the Board on a case-by-case basis that is deemed by the Board to be substantially equivalent to the requirements.
